Use of the Service

The provider websites provide general information about a Practice and allow visitors to submit inquiries, book appointments, or learn about services. The provider websites do not constitute medical advice; a clinical relationship is established only after you become a patient of the Practice under its standard intake process.

Account & Information You Provide

You agree that the information you submit through a provider website's forms is accurate. You are responsible for safeguarding any credentials you create. Information you submit is handled according to the Privacy Policy.

Prohibited Conduct

• Do not use the provider websites to transmit unlawful, fraudulent, or infringing content.
• Do not attempt to interfere with, disable, or circumvent any security, authentication, or rate-limiting controls.
• Do not impersonate any person or misrepresent your affiliation with any person or entity.

Intellectual Property

The Vye Health platform, including templates, software, and underlying technology, is owned by Vye Health or its licensors. Practice-specific content (logos, photos, bios) belongs to the Practice.

The provider websites are provided "as is" without warranty of any kind. Vye Health is not responsible for the clinical decisions of any Practice, nor for the accuracy of Practice-supplied content.

Limitation of Liability

To the maximum extent permitted by law, Vye Health is not liable for any indirect, incidental, or consequential damages arising from your use of the provider websites.
